Membershipform Twente '05

The club fees (annual membership fee, possible competition costs, and the yearly cost of the competition shirt) are collected in 4 terms (End of September, End of November, End of Januari, End of March; approximately every 8 to 9 weeks). One-time costs (registration fees and first purchase of the competition shirt) are collected in one go, combined with the first term.

This registration form also serves as a direct debit authorization. The undersigned (or the parent/guardian) hereby grants permission for the automatic deduction of the one-time costs and the club fees as long as the undersigned remains a member of the club, in compliance with the membership conditions (Dutch).

De vereniging publiceert teamindelingen met teamfoto(s) op de verenigingswebsite(s). Elk lid kan in het kader van de AVG aangeven of hij/zij bezwaar heeft tegen publicatie van voornaam en/of foto.

The choices are:
- Complete: Your first name will be displayed on the website and you also don't mind being in a team photo or on a photo published on our social media channels.
- Limited: Your first name may be published but you prefer not te be in a team photo nor  a photo featuring you published on our social media channels.
- Nowhere: Your first name will not appear on the team list, and no photo featuring you will be published.
